FORMS AND APPLICATIONS FOR DSC 2012 AND NEWLY APPOINTED TEACHERS

Useful forms and applications for DSC 2012 teachers.
All the Newly Appointed Teachers have to submit the Physical Fitness form before the Mandal Educational Officer or Gz.Head Master (if High School) to get appointment order proving that he/she is fit for the post.

EMPLOYEE ID (TREASURY ID) :The newly appointed teachers (DSC 2012) have to fill two sets of Employee Id proposal form for allotment of Treasury Id Number for the use of Salaries which will be their unique identification number.

CPS-PRAN: The newly appointed teachers also have to fill the PRAN application form for allotment of PRAN Number for their CONTRIBUTORY PENSION SCHEME Deductions.

APGLI:  Andhra Pradesh Government Life Insurance subscription is compulsory for all Employees. They have to subscribe under allotted slabs for them. The subscription form should be filled after deduction of one month subscription towards APGLI
